The website that offers the Moderna vaccine is a partnership between Passport Health and the Mollen Foundation. Proof of age required.

The county is also working to set up vaccination events for people who meet age requirements in the coming weeks.

Individuals 75 and over who fill out an online form will be notified when events are scheduled in their area.

There are already 20 grocery pharmacies on Metro Phoenix that offer COVID-19 vaccines for people aged 75 and over.

Maricopa County works with Valley Hospitals to operate five dispensaries for all eligible priority populations and one at Grand Canyon University.

The county is still in the Phase 1B priority phase for vaccine approval. The age limit is 75 years.

The state operates two mass vaccination sites in the region, the State Farm Stadium in Glendale and the Phoenix Municipal Stadium, which are in the same stage but have a lower age requirement of 65 years.
